Registered Nurse - Community Outreach

Seeking a registered nurse to assess, manage, and coordinate patient care during hospital stay, utilizing a patient-centered approach. Key Responsibilities Include Performing comprehensive assessments, documenting findings, and evaluating patient needs. Developing and adjusting individualized care plans based on assessments, diagnoses, lab data, and physician orders. Administering medications, monitoring IV fluids, providing patient education, and coordinating referrals and discharge planning. Recognizing changes in patient condition, implementing appropriate nursing actions, and ensuring effective communication with the healthcare team. Delivering specialized care such as restraint management, pain control, and abuse assessment, while adhering to safety and infection control protocols. Maintaining professional accountability through ongoing education, quality standards, and compliance with policies. Providing compassionate, respectful service, ensuring patient privacy, and fostering collaborative teamwork. Participating in performance improvement initiatives, evidence-based practice, and research activities. Qualifications And Skills Current RN license in applicable states with required certifications (e.g., BLS). Ability to perform age-appropriate care for adult and geriatric patients. Strong communication, critical thinking, and teamwork skills. Commitment to safety, infection control, and professional development.
