The Wellness Nurse LVN performs his/her duties under the supervision of the SNF Resident Wellness Director while adhering to mandated nursing standards. The Wellness Nurse LVN is responsible for maintaining compliance with all state and federal regulations while utilizing the standards of care policies and procedures.ESSENTIAL JOB FUNCTIONS:Servant Leadership directs all aspects of decision makingContributes to all aspects of medication management, including but not limited to; orders, administration, storage, documentation and as mandated by the State Regulations and Guidelines as well as Company policy and procedureAssures resident/patient rooms and common areas are clean and safe at all timesAssure that all resident/patient care specialists are familiar with the residents'/patients' individual conditions and are trained to provide the resident services in an effective, professional and sensitive mannerContinually assess residents/patients to identify any change in condition, mood, behavior, or level of function to ensure appropriate interventions to maintain the residents' highest practicable physical, mental and psychosocial wellbeingAdministration of medicationsImplements plans of care recommended by the Resident Wellness DirectorImplements change in condition plansServes as the "Charge Nurse" for assigned shift ensuring residents/patients are well cared forProvides accurate and timely documentation of all care provided to residentsAssembles and maintains resident charting and accurately maintains all recordsCompletes accident and incident reports as requiredMaintains relationships with residents, family members and responsible partiesAssist in the coordination of effective communication among the team, residents/patients, and familiesBe prepared for and manage emergency situations including disasters, fire and other emergenciesEnsures adequate and appropriately trained team members are available for each shift within company and state guidelinesMust be in company uniform and resident ready at all timesPerforms other related essential duties as assignedKNOWLEDGE, SKILLS AND ABILITIES:Able to communicate effectively with all levels of management, team members, residents/patients, family members, vendors, referral sources and outside contactsAble to make independent decisionsMust be able to communicate in a warm, friendly and caring mannerMust be familiar with and adhere to guidelines related to the Fair Housing Act (FHA) and the American with Disabilities Act (ADA)Must possess a passion to work with and around senior citizensKnowledge of customer service principles and practicesEDUCATION REQUIREMENTS:LVN licenseKnowledge of the physiology and psychology of the older adultEXPERIENCE PREFERENCES:Minimum One (1) year in the senior living environmentPHYSICAL REQUIREMENTS:The following physical demands are representative of those that must be met to successfully perform the essential functions of this position:Able to stand or walk 75% of the dayAble to concentrate with frequent interruptionsAble to work under stress and in emergency situationsAble to work under conditions that require sitting, standing, walking, reaching, pulling, pushing, and graspingAble to talk and hear effectively in order to convey instructions and information to residents and team membersOccasionally lift/carry up to 50 poundsOCCUPATIONAL EXPOSURE / PERSONAL PROTECTIVE EQUIPMENT:Work in all areas of the communitySubject to reactions from dust, disinfectants and cleaning chemicals and may be exposed to infectious waste, diseases and other conditionsUse personal protective equipment and supplies when needed to prevent burns, falls, and infectionPersonal protective equipment includes infection control kit, rubber gloves, masks and non-slip shoesSubject to infectious diseases, substances and odorsFollow Safety Policy & Procedures10:00pm-6:00am, Rotating, Skilled Nursing, Must have 1 year experience as LVN/RN.
