- Provide patient care and assist in daily living activities.
- Administer medications and treatments as prescribed by the healthcare provider.
- Monitor patients' health status and report changes to the nursing staff.
- Document patient information and care provided accurately.
- Collaborate with healthcare team members to ensure quality patient care.
- Current LPN/LVN license in the state of Pennsylvania.
- Experience in long term care preferred.
- Basic life support (BLS) certification required.
