RN Clinical Liaison: Care Coordination & Referrals

Ochsner Health seeks an RN for clinical coordination of care to manage referrals into the home infusion/home health care setting. You will identify patient needs, educate families, and expedite discharge planning with hospital teams. The role requires an RN license in the applicable state and 3+ years nursing experience, with strong communication and computer skills for data entry and care coordination.
