Home Doctors for You is an established home visiting physician serving homebound patients and complex geriatric patients across Chicagoland and the surrounding area. Our clinical team completes 500–600 patient encounters per month.

We are seeking an experienced, independent Certified Medical Biller & Coder (1099 Contract) to handle daily chart review, coding, superbill creation, clearinghouse management, and denial resolution.

Position Overview:

• Status: Independent Contractor (1099)

• Hours: Part-Time, roughly 18–22 hours per week (capped at 25 hours/week)

• Schedule: Flexible daytime hours, Monday through Friday

• Pay: $35.00 – $42.00 per hour (based on home-based primary care experience)

• Location: 100% Remote (US-based only; Illinois Medicaid / HFS experience preferred)

Key Responsibilities:

• Review signed clinical SOAP notes in Practice Fusion EHR and assign ICD-10-CM and CPT/HCPCS codes from scratch.

• Code and bill complex home/domiciliary visits (Place of Service 12 & 13; CPT 99341–99350) with appropriate E/M medical decision-making levels and modifiers (e.g., -25).

• Code and bill care management services: Transitional Care Management (TCM 99495/99496), Chronic Care Management (CCM 99490/99439), Care Plan Oversight (CPO G0181/G0182), Advance Care Planning (ACP 99497), and Annual Wellness Visits (AWVs).

• Generate electronic superbills in Practice Fusion and transmit batch ANSI 837P files to Office Ally / Availity clearinghouse.

• Work clearinghouse front-end edits (999/277 reports) and correct rejections within 24 hours.

• Reconcile 835 ERAs, post payments/adjustments, and monitor Medicare Part B crossovers to Illinois Medicaid (HFS/IMPACT) for our ~30% dual-eligible census.

• Appeal clinical and technical claim denials; keep practice Days in A/R under 32 days.

Qualifications & Requirements:

• Active AAPC or AHIMA certification (CPC, CPB, or CCS-P) in good standing (Required).

• Minimum 2+ years of specialized experience billing for Home-Based Primary Care (HBPC), house calls, or mobile medical practices using POS 12/13.

• Demonstrated mastery of Medicare Part B rules (NGS MAC) and Medicare crossover / QMB non-billing compliance.

• Proficiency with Practice Fusion EHR and clearinghouse portals (Office Ally or Availity).

• Must sign a standard Business Associate Agreement (BAA) and contractor agreement.
