Job Title: Targeted Case Manager Position Summary At Lumera Healthcare, we believe healing happens through connection, collaboration, and compassionate care. We are seeking a dedicated Targeted Case Manager (TCM) who is passionate about empowering individuals to overcome barriers, strengthen natural supports, and achieve greater independence. Targeted Case Managers serve as advocates, care coordinators, and resource navigators for individuals living with serious mental illness (SMI), serious emotional disturbance (SED), and substance use disorders (SUD). Through comprehensive assessment, service coordination, and ongoing support, our TCMs help clients access medical, behavioral health, housing, educational, vocational, and community resources that promote long-term recovery and wellness. This position manages a caseload of up to 25 clients while working closely with Lumera's multidisciplinary team to deliver integrated, person-centered care. Approximate Allocation of Time 50% Direct Client Services 25% Documentation 15% Professional Development & Community Resource Building 10% Administrative Responsibilities Essential Responsibilities Client Care & Service Coordination Accept referrals following completion of a diagnostic assessment by a Lumera Healthcare provider. Complete comprehensive Targeted Case Management assessments and required reassessments in accordance with Kentucky Medicaid regulations. Collaborate with clients to develop individualized, strengths-based treatment plans that reflect their goals, preferences, and identified needs. Provide ongoing Targeted Case Management services that promote stability, recovery, and increased independence. Monitor client progress and revise treatment plans as needed to address changing circumstances or emerging needs. Coordinate care with internal providers and external community partners to ensure continuity of services. Connect clients with appropriate community resources including housing, healthcare, transportation, education, employment, financial assistance, and other supportive services. Advocate for clients while promoting self-advocacy and increased independence. Collaboration Maintain regular communication with therapists, psychiatric providers, medical staff, intake coordinators, and other members of the interdisciplinary treatment team. Develop and maintain positive working relationships with community agencies and referral partners. Participate in treatment team meetings, case consultations, and weekly staff meetings. Documentation & Compliance Complete timely, accurate, and clinically appropriate documentation, including: Assessments Treatment plans Progress notes Ensure documentation complies with Kentucky Medicaid regulations, accreditation standards, and Lumera Healthcare policies. Follow all mandatory reporting laws regarding abuse, neglect, and exploitation. Professional Responsibilities Demonstrate effective interviewing, assessment, engagement, and motivational skills. Utilize crisis intervention, de-escalation techniques, and evidence-based approaches to support client success. Prioritize workload and independently manage a caseload of up to 25 clients. Continue building knowledge of community resources and behavioral health best practices. Perform other duties as assigned that support Lumera Healthcare's mission and clinical operations. Qualifications Required Bachelor's degree in: Social Work Psychology Sociology Human Services Or another related behavioral health field Eligibility to provide Targeted Case Management services under Kentucky Medicaid regulations. Completion of Kentucky Targeted Case Management training within six months of hire and ongoing maintenance of certification requirements. Excellent interpersonal, organizational, and communication skills. Strong technology skills, including proficiency with electronic health records and virtual communication platforms. Ability to work independently while collaborating effectively within a multidisciplinary team. Reliable transportation, valid driver's license, and willingness to travel within the service area as needed. Preferred Experience working with individuals diagnosed with serious mental illness, substance use disorders, or co-occurring disorders. Knowledge of community behavioral health resources and recovery-oriented systems of care. Experience in care coordination, case management, or community mental health settings. Why Lumera Healthcare?
