Home Health Licensed Practical Nurse

About the Role As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients return to the life they love. Responsibilities Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and their families on disease management, treatment options, and self‑care techniques. Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes. Maintain timely communication with patients, physicians, clinical manager(s), and other team members regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning. Required Qualifications Current nursing license in the practicing state. Valid driver’s license, auto insurance, and reliable transportation. Current CPR certification. Two years of experience as an LPN/LVN in a clinical setting. Preferred Qualifications Experience in a Home Health or Hospice setting. Compensation and Hours Pay Per Visit/Unit Rate: $33.00 – $47.00. Scheduled Weekly Hours: 40. Pay Range: $53,000 – $73,000 per year.
