Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Requirements:

• Active California RN license (required)
• Current BLS certification through the American Heart Association
• Minimum 5 years of recent RN Case Management experience
• Experience in LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) settings
• Proficiency with MCG (Milliman Care Guidelines) and discharge planning processes
• Demonstrated success in interdisciplinary team collaboration
