Location: Jacksonville, Florida | Employment Type: Full-Time, W-2 | Compensation: $115,000 – $145,000+

What You'll Do

You will provide comprehensive patient centered care across both family medicine and pain management service lines in a fast paced collaborative environment. On the family medicine side you will conduct thorough patient assessments, diagnose and manage acute and chronic conditions, develop personalized treatment plans, and deliver preventive care and chronic disease management for conditions including diabetes and hypertension. On the pain management side you will evaluate and treat patients with a variety of pain conditions, collaborate on multimodal treatment plans under physician supervision, monitor therapies, ensure compliance, and coordinate care across the team for optimal patient outcomes.

Qualifications

  • Master's or Doctorate degree in Nursing from an accredited program - required
  • Active Florida Nurse Practitioner license - required
  • National certification - FNP or equivalent - required
  • Current DEA registration - required
  • Minimum 2 years of NP clinical experience - required
  • Excellent diagnostic, communication, and EMR skills - required
  • Prior experience in busy Family Medicine or Pain Management settings - preferred
  • Familiarity with value-based care - preferred
  • Bilingual abilities Spanish/English - highly valued
