This role offers the opportunity to function as an Ambulatory Clinical Pharmacy Specialist providing comprehensive clinical pharmacy services within an ambulatory care setting. Operating as a credentialed and privileged member of the healthcare team, the pharmacist will work under Collaborative Practice Agreements to optimize therapeutic outcomes, improve medication access, and support cost-effective care delivery. This position emphasizes collaboration with providers, nurses, and multidisciplinary teams to develop, implement, and monitor therapeutic plans grounded in safe and evidence-based medication use.

Responsibilities include:

• Delivering ambulatory clinical pharmacy services per Collaborative Practice Agreements, including therapeutic plan assessment, monitoring, and documentation
• Conducting patient visits focused on medication management, reconciliation, laboratory monitoring, counseling, and disease state management
• Collaborating with providers to implement evidence-based medication interventions that enhance quality, safety, and reduce costs
• Managing referrals with communication and follow-up to referring clinicians
• Coordinating prior authorizations, financial assistance programs, and benefit investigations to support patient access and affordability
• Providing clinical oversight and support for specialty medication therapy initiation and monitoring
• Offering drug information and education to patients, families, and healthcare staff
• Precepting pharmacy students, interns, and residents as assigned
• Participating in system improvements, research, committee workgroups, and quality initiatives related to ambulatory pharmacy services
• Documenting all interventions in the electronic health record to meet organizational and national practice standards

Qualifications:

Must Have:

• Active Delaware Pharmacist License in good standing
• Board of Pharmacy Specialties (BPS) certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (required)
• Minimum one year of clinical pharmacy experience, including residency experience
• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree

Preferred:

• Doctor of Pharmacy (PharmD) preferred
• Completion of PGY1 or PGY2 ambulatory care residency strongly preferred
• Experience with Collaborative Practice Agreements and specialty medication management
• Familiarity with Comprehensive Medication Management (CMM), chronic disease management, or population health pharmacy
• Prior authorization and financial assistance experience
• Preceptor experience
