Job Description

Social Worker to provide psychosocial support and care coordination for patients receiving outpatient dialysis treatment. The role focuses on assessing patient needs, assisting with care planning, coordinating community resources, and supporting patients and families throughout their treatment journey. This position does not involve clinical therapy or counseling.

The Social Worker will primarily support dialysis patients and may be required to travel or conduct telehealth visits for additional outpatient locations as needed.

• Minimum 1 year of Social Work experience
• MSW or LCSW required
• Active North Dakota Social Work Licensure
• Current BLS Certification
• Experience in outpatient or healthcare settings preferred
• Previous travel experience is not required
• Comfortable working independently and collaborating with multidisciplinary teams
• Willingness to travel to additional outpatient locations as needed

Responsibilities

• Conduct psychosocial assessments for dialysis patients
• Assist patients with discharge planning and resource coordination
• Provide emotional support and patient education
• Coordinate with physicians, nurses, and interdisciplinary care teams
• Connect patients with community resources and financial assistance programs
• Maintain accurate documentation in Epic EMR
• Manage a varying patient caseload (approximately 95 patients per month)
