Roles & Responsibilities

• Screen, evaluate, and treat patients to help them regain function and mobility
• Develop and update personalized treatment and discharge plans of care
• Document services accurately and timely to ensure continuity of care
• Supervise Physical Therapy Assistants and Rehab Techs, providing guidance per practice acts
• Collaborate with interdisciplinary care teams and families to optimize patient outcomes
• Maintain compliance with safety, infection control, and regulatory standards

Qualifications

• Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT)
• Active Illinois Physical Therapy license or ability to obtain one before start
• Strong communication, documentation, and clinical reasoning skills
• Flexibility to adapt to varying facilities, schedules, and patient needs
