North Central Public Health District is hiring a Home Visiting Nurse to join our team in The Dalles, Oregon.

Key Details:

Pay: $78,000 - $87,750 annually (Exempt), depending on experience. Our workweek is 37.5 hours.

Benefits: Medical, dental, vision, retirement, PTO, and holidays

Location: 419 East 7th Street, The Dalles, OR 97058, with home visits across Wasco and Sherman Counties in a district vehicle and some remote time for charting and administrative work.

Schedule: Monday through Friday, generally 8:30 am to 5:00 pm with an hour lunch, and real flexibility on when your day starts and ends depending on your individual needs.

Who You Are:

You're a nurse who's comfortable working at a family's kitchen table. Home visiting runs without the standing orders and protocols of an inpatient floor, so you bring your own judgment and your own plan. You've worked with parents, babies, or kids somewhere along the way, and you're comfortable in homes that don't look like yours. We can teach you home visiting. What we're looking for is the clinical knowledge and the temperament.

Other qualifications include:

Licensure: Active RN or BSN

Clinical background: Maternal and child health experience of any kind, including in a hospital. Public health and ambulatory nursing backgrounds move into this work most easily.

Judgment: Comfort solving problems without a protocol, and running your own visit schedule against a monthly target.

Community focus: A commitment to equitable care for families who haven't always had access to it.

Driving: A valid driver's license and a driving record that clears our review, so you can use our fleet vehicles.

Screening: Offers are conditional on a background check, driving record review, license verification, and references.

What You'll Do:

A new grant lets us open home visiting to any family with a baby, regardless of insurance, where before we could only serve Medicaid clients. That means more families and in-home lactation support on top of what we already do. You'll carry your own caseload alongside a community health worker who sees your families between your visits, and you'll help build a program that's growing.

A typical day will consist of:

Visiting families in their homes to assess, teach, and build care plans for parents, babies, and children with special health care needs.

Providing in-home lactation support to new parents.

Working as a pair with a community health worker who handles resource connection between your visits.

Charting and prep, which runs about an hour and a half for every hour of direct client care.

Running your visit schedule against a monthly goal. Our lead nurse is expected to see about 7 visits a week, and weeks vary: 10 visits one week, mostly documentation the next.

Driving a district fleet vehicle across Wasco and Sherman Counties. Some visits are an hour out, and that drive time is paid work time.

Joining the home visiting team meeting every other week and case meetings with your community health worker.

Taking part in community of practice work with the state and partners around Oregon.

Other duties as assigned.
