Home Care Coordinator – All Connecticut Home Care Services

All Connecticut Home Care Services is seeking a compassionate, motivated, and organized Home Care Coordinator to join our fast-growing team. This position is ideal for someone with strong communication and organizational skills and a genuine passion for serving the disabled and elderly communities.

The ideal candidate will have experience with client intake, staff recruitment and supervision, and coordination of care, along with a working knowledge of Medicaid and private-pay systems. Prior experience working in a homemaker and companion care setting is required. Serious inquiries only.

Key Responsibilities

• Serve as the primary point of contact for Medicaid referrals and client intake.

• Possess a minimum of two (2) years of experience working within a homemaker and companion agency.

• Demonstrate familiarity with community-based Medicaid programs , including referrals from community care programs and the Agency on Aging .

• Support marketing and business development efforts to grow the private client base, including outreach to retirement communities, veterans’ homes, and other referral sources.

• Coordinate, maintain, and manage client files for both Medicaid and private-pay clients in full compliance with agency policies and state regulations.

• Recruit, interview, and hire qualified home health aides, including Certified Nursing Assistants (CNAs) and Personal Care Assistants (PCAs) .

• Supervise caregivers to ensure high standards of client care and satisfaction.

• Monitor staffing needs and conduct required background checks for prospective employees.

• Maintain accurate and up-to-date electronic records.

• Conduct client outreach and follow-up to ensure ongoing satisfaction with services provided.
