Job Summary:

LPN Licensed Practical Nurse Responsibilities:
Provide high-quality, individualized care to patients in long-term care, rehabilitation, and skilled nursing facilities
Collaborate with interdisciplinary teams to develop and implement comprehensive care plans, ensuring seamless transitions and optimal patient outcomes
Conduct thorough patient assessments, monitor and report changes, and communicate effectively with patients, families, and healthcare professionals
Administer medications, treatments, and procedures as prescribed by physicians and within the scope of practice
Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices
Foster a positive, supportive environment by promoting patient dignity, respect, and comfort
LPN Licensed Practical Nurse Requirements:
Current LPN license in the State of New York
Strong communication and interpersonal skills
Commitment to providing patient-centered care and contributing to a positive, collaborative environment
