Job Description
Hospice Case Manager

Position Overview

The Hospice Case Manager is responsible for delivering exceptional care to patients in a home health setting. This role requires specialized experience in hospice care to effectively manage patient caseloads and coordinate comprehensive care plans tailored to individual needs.

Key Responsibilities

Provide hospice nursing care in patients' homes, ensuring comfort and support for patients and their families.
Develop and implement personalized care plans based on thorough assessments and ongoing evaluations.
Collaborate with interdisciplinary teams to facilitate seamless communication and coordination of care.
Educate patients and their families on disease progression, care options, and available resources.
Document all patient interactions and care provided in compliance with regulatory standards.
Ensure adherence to hospice policies and procedures to maintain quality of care.

Qualifications

Current and valid Registered Nurse (RN) license.
Previous experience in hospice care within a home health environment is required.
Strong communication and interpersonal skills to engage effectively with patients and families.
Ability to work independently and manage multiple priorities in a fast-paced setting.
Familiarity with electronic health records (EHR) and documentation standards.

Work Schedule

This position typically involves day shifts, working five 8-hour days from 08:00 AM to 04:30 PM, with no call requirements.

Compensation

Mileage will be reimbursed at the current IRS rate for travel between patient locations.
