Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, tailoring treatment plans to meet individual needs
• Deliver effective physical therapy interventions that enhance mobility, strength, balance, and overall function
• Collaborate closely with a multidisciplinary team, including nurses, physicians, case managers, and fellow rehabilitation professionals, to optimize patient outcomes
• Educate patients and their families about therapeutic objectives, discharge planning, and essential home exercises
• Maintain meticulous documentation of evaluations, treatments, and patient progress in line with facility and regulatory requirements

Education

• Doctorate in Physical Therapy (DPT) or equivalent from a recognized program

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
