Physical Therapist | UF Health Gainesville Homecare | PRN

Job Title

Provision of professional therapy services in compliance with the state Practice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and Shands HomeCare policies, procedures, and philosophy of care. The Physical Therapist is responsible to proactively apply professional knowledge and skills to meet the needs of the patient/client and facilitate the achievement of individualized, defined and measurable outcomes. This position reports to and is supported by the Director of Rehab or designee. Reporting to this position may be a Physical Therapy Assistant.

Must successfully pass an AHCA Level 2 screening for this position.

Qualifications

Minimum Education and Experience Requirements: Graduate of an accredited school of Physical Therapy with a Bachelors, Masters or Doctoral Degree. Licensed in the State of Florida. Home care experience preferred.

Motor Vehicle Operator Designation: Employees in this position: Will operate vehicles for an assigned business purpose as a 'frequent driver'. Note: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
