Role Description This is a full-time, hybrid Home Health Licensed Practical Nurse (LPN) role based in Lakewood, CO, with some work-from-home flexibility for documentation and care coordination. The LPN will provide skilled nursing care in patients’ homes, including assessments, medication administration, wound care, and monitoring of vital signs and symptoms under the direction of a Registered Nurse and physician orders. Daily responsibilities include coordinating care plans, educating patients and caregivers on disease processes and treatments, and promoting safety and independence in the home environment. The LPN will document all visits and clinical findings accurately in the electronic health record, communicate changes in condition promptly to the interdisciplinary team, and participate in case conferences. The role involves traveling to patient homes, collaborating with home health and hospice team members, and upholding regulatory, agency, and infection control standards. Qualifications Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Colorado; current BLS/CPR certification. Strong Nursing and Medicine knowledge, including vital signs monitoring, medication administration, and basic clinical procedures. Experience in Home Care and Patient Care, preferably in home health or hospice, with the ability to work autonomously in varied home settings. Excellent assessment, critical thinking, and documentation skills; proficiency with electronic health records and basic computer tools. Valid driver’s license, reliable transportation, and ability to travel to patient homes within the service area. Effective communication and interpersonal skills to work with patients, families, and an interdisciplinary team in a culturally sensitive manner. Ability to manage time, prioritize visits, and maintain professional boundaries while providing compassionate, patient-centered care.
