Full-Time Nurse Practitioner / Physician Assistant Post-Acute

We are seeking a compassionate and skilled Post-Acute Advanced Practice Provider (APP) to join our care team. In this role, you will support patients and families as they transition from hospital to post-acute settings, managing recovery and complex health needs with expertise and empathy.

About the Role: The APP will provide comprehensive medical care, symptom management, and emotional support, working to enhance comfort, promote healing, and improve quality of life for every patient throughout their recovery journey.

Compensation: In compliance with New Jersey pay transparency laws, all clinician roles include a posted minimum base salary. This role offers a base starting salary at $125,000.00. Final compensation will be competitive and commensurate with experience, qualifications, and prevailing market factors.
