Registered Nurse - Hospice

We are seeking a Registered Nurse Case Manager to deliver comprehensive hospice care, develop personalized Plans of Care, and coordinate treatment strategies that support patients' physical, psychosocial, emotional, and spiritual needs. Key responsibilities include conducting initial eligibility evaluations, reassessing patient needs at least every 15 days, documenting care progress, collaborating with patients, families, physicians, and interdisciplinary teams, and providing education on self-care techniques. Applicants must hold a current, unrestricted RN license in the practice state and have reliable transportation.
