Our Registered Nurse (RN) Case Manager plays a vital role in ensuring patients receive coordinated high-quality care throughout their healthcare this role you will monitor and manage patient care to promote continuity optimize outcomes enhance patient satisfaction and support cost-effective compliant care delivery. Working closely with physicians nurses payors community partners and other members of the multidisciplinary team you will lead discharge planning and utilization review activities while advocating for patients and connecting them with the resources they need for a safe and successful transition of care. How youll contribute

A RN Case Manager excels in this role:

• Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care including admission length of stay transfers and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints from patients families and payors.
• Leads and coordinates discharge planning including patient transfers to other facilities and referrals to community resources; provides discharge education.
• Performs chart reviews to identify and address issues related to service delivery patient outcomes and satisfaction compliance cost and reimbursement.
• Communicates effectively with providers payors and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities plans and outcomes.
• Participates in performance improvement and required continuing education.
• Supports department operations by working alternate shifts/units as needed; may be assigned on-call.
• Performs other duties as assigned.

What were looking for

Applicants should have a current state or compact state RN license and possess an associates degree from an accredited nursing school. Additional requirements include:

• Certifications: BLS
• Previous MCG experience preferred
• Licenses: Must have a current state or compact state RN license.
• Education: Associates degree required. BSN preferred.
