They supervise and guide pharmacy support personnel to ensure safe and effective operations. At Cary, youll be part of a supportive, collaborative team where your expertise and contributions truly make a difference./ppEssential Job Duties, but not limited to:/pulliAbility to work independently with minimal direct supervision./liliAbility to work cooperatively with health-system and pharmacy staff./liliAbility to handle frequent interruptions and adapt to changes in workload and work schedule./liliAbility to set priorities makes critical decisions, and responds quickly to emergency requests./liliAbility to exercise sound professional judgment./liliAbility to communicate effectively (orally and in writing)./liliAbility to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncology, obstetric, rehabilitation, and skilled-nursing patients./liliInterprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles./liliPrepares and dispenses medications, including sterile, chemotherapy, and parental nutrition preparation safely and accurately using appropriate techniques./liliIssues controlled substances to patient care areas and maintains records as required by law./liliMonitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications./liliAssists with pharmacokinetics consult service and drug dosing per organizational protocol./liliReviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed./liliReads, extracts, and interprets information in patient charts accurately./liliDetects and reports suspected adverse drug reactions and medication variances accurately and in a timely manner./liliSustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational medication therapy selection./liliProvides clinical consultation and clarification to practitioners. Recommends evidence-based medication therapy regiments and monitoring plans. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed./liliProvides accurate, adequate, and timely drug information to the professional staff./liliProvides drug information to patients and their families./liliDocuments all clinical activities and interventions accurately and completely./liliParticipates in the quality improvement and medication-use review activities of the department.
