OT Home Health Contract – Largo/St. Petersburg, Florida

Ardor Health Solutions is seeking an experienced Occupational Therapist for a Home Health contract opportunity with BayCare HomeCare in the Largo and St. Petersburg area.

In this role, you'll provide skilled therapy services to adult and geriatric patients in their homes, helping them improve independence, safety, and overall quality of life. Patient populations may include orthopedic, cardiac, neurological, pulmonary, and medically complex individuals requiring skilled therapy intervention. Responsibilities include evaluations, ADL training, home exercise programs, transfer training, patient and caregiver education, safety assessments, and discharge planning.

Requirements

• Active Florida Occupational Therapist license
• Minimum 1 year of Home Health experience required
• Homecare Homebase (HCHB) experience required
• OASIS experience required
• Current driver's license and reliable transportation
• Strong communication and documentation skills
