Registered Nurse Case Manager, Level 2

Full Time Registered Nurse Case Manager, Level 2 Anticipated Territory: Citrus Heights and surrounding cities. What you’ll do as a Registered Nurse (RN), Level 2 Home Health Case Manager: Utilize your clinical skills to create, implement, and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes. Oversee 2 nurses responsible for executing your care plan. Conduct comprehensive patient assessments such as Start of Care (SOC), Evaluations, Resumption of Care and discharge planning. Assist and teach patients and their family/caregivers with wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication education in the patient’s home. Perform a full assessment of the patient & family upon initial visit. Use assessment data to develop an advanced plan of care including rehabilitation. Work with patients and family one-on-one to achieve improvement and stability. Case manage patient care episode with interdisciplinary clinical team. Prepare clinical notes, orders, and charting within a compliant time frame. Collaborate with physicians and agency staff to deliver the best care possible. To be a success you need One year of home health case management experience preferred. Minimum of one year nursing experience required; two+ years preferred. Experience supervising and mentoring nurses preferred. Current and unrestricted California RN license. Current BLS/CPR certification. Valid California driver’s license. High level of clinical knowledge in home health care. Experience with high-acuity patients is a plus. Experience working with an interdisciplinary team required. Case management fundamentals required. Self‑directed, empathetic, critical thinker. Outstanding interpersonal skills, leadership, organized. Compensation Estimated annual salary: AMD140,000 - AMD160,000 plus up to AMD20,000 annual bonus paid quarterly. Starting pay commensurate with experience.
