Position Summary We have an exciting opportunity to join our team as a Per Diem MSW Social Worker. The successful candidate will work under the supervision of a Social Work Supervisor or Director, providing psychosocial assessment and counseling services, including discharge planning to patients assigned by the Department of Social Work/Care Management and under Social Work supervision. The role also involves providing information and education to patients and families pertaining to patient needs and discharge plans, and supervising social work interns. Job Responsibilities Perform other duties as assigned. Provide timely and effective psychosocial assessments and evaluations, using clinical knowledge of psychosocial dynamics and interviewing techniques to gather social history, define problems, and develop treatment plans. Coordinate interdisciplinary information and recommendations into social work treatment plans and establish post‑hospital/discharge plans. Implement plans for psychosocial assessments and evaluations as required. Use appropriate interventions according to patient/family dynamics and needs, considering a variety of approaches. Develop and conduct group programs as appropriate to assigned services, maintain a therapeutic and supportive relationship with patients and families, and use professional judgment and creativity in seeking and presenting alternative or innovative solutions. Document all social work activities in a timely manner according to department protocols, maintain charting on HIS systems or outpatient records, and record clear, legible, and concise assessments of cultural and spiritual needs. Perform clinical duties in an independent manner with appropriate use of supervision, share pertinent information with the interdisciplinary team, and perceive the needs of the service unit and department, offering suggestions or recommendations for positive change. Organize work to achieve efficiency and complete assignments, demonstrate flexibility in adapting to change, and maintain accessibility via telephone or beeper during the workday, checking and following up all incoming messages and written communications. Demonstrate knowledge of resources and procedures related to discharge planning, identifying and mobilizing community resources appropriate for implementation of discharge plans, and coordinating with the Discharge Planning Unit and other services in hospital and community. Demonstrate skill in presentation of psychosocial information at interdisciplinary rounds, meetings, and Rusk evaluation conferences. Participate actively and effectively in the presentation of psychosocial information and discharge planning at rounds and evaluation conferences, and contribute to staff and departmental development by serving regularly and productively on staff committees or hospital committees. Maintain reliability by adhering to attendance and punctuality guidelines, departmental guidelines regarding time off, employee health requirements for annual health assessment, TB screening, and proof of all required immunizations. Maintain a professional appearance and wear identification badge at all times while at the Medical Center. Minimum Qualifications Master’s Degree in Social Work, or an equivalent higher degree from a Social Work program registered by the Department as licensure qualifying, accredited by the Council on Social Work Education (CSWE) or determined by the Department to be substantially equivalent to a registered program. Licensed Master Social Worker (LMSW) licensure or a New York State Education Department‑issued limited permit, with completion of coursework or training in the identification and reporting of child abuse in accordance with Section 6507(3)(a) of the Education Law. Required Licenses Licensed Social Worker – NYS‑Permit, Licensed Social Worker – NYS. Preferred Qualifications Previous experience in a hospital or health‑related organization. Qualified candidates must be able to effectively communicate with all levels of the organization.
