Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, following the care plan and physician orders for each patient while maintaining confidentiality and professional boundaries. Responsibilities Deliver and coordinate quality patient care in compliance with physician orders. Continuously observe and assess patient condition and care needs, reporting changes to supervisor and/or physician. Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner. Participate in, implement, and update the nursing care plan. Take appropriate nursing action based on assessment to achieve expected outcomes. Recognize changes in patient needs and responses requiring intervention, implementing care to prevent or reduce risk. Accept responsibility for personal and professional accountability by complying with company policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act. Provide care utilizing infection control measures that protect staff and patients per OSHA standards. Educate patients and families about disease processes, self-care techniques, and prevention strategies. Maintain knowledge of competency related to the nursing profession through ongoing education and skill evaluations. Qualifications Graduate of an accredited nursing school. Current, unrestricted state license as an LPN/LVN in the state of practice. Current CPR certification. Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies. Additional State Specific Requirements South Carolina – One year of pediatrics experience. California – One year of experience under a current nursing license. Louisiana – One year of experience as a licensed nurse. Continuing Education as required by state. Preferences Six months recent experience as a Licensed Nurse in a clinical setting. Home health experience. Other Skills & Abilities Attention to detail. Time management. Effective problem‑solving and conflict resolution. Good organization and communication skills. Physical Requirements Must be able to speak, write, read, and understand English. Must be able to travel. Must be able to lift 50 pounds. Must be able to reposition patients and move equipment without assistance. Prolonged walking, standing, bending, kneeling, reaching, twisting. Must be able to sit and climb stairs. Must have visual and hearing acuity. Must have a strong sense of smell and touch. Must be able to respond physically and mentally to emergency situations at home or during transport.
