Quality Outreach Coordinator

In this vital role, you will be part of a dynamic remote care team—partnering alongside fellow Patient Care Advocates, Quality Care Specialists, and Practice Coordinators—driving seasonal outreach campaigns to close critical care gaps. Your daily focus will involve connecting directly with members and healthcare providers, facilitating preventive health compliance, and accurately documenting assessments within internal healthcare systems. The ideal candidate brings a solid foundation in managed care, hands-on familiarity with HEDIS quality measures, Medicare standards, and healthcare claims, along with a compassionate, solution-oriented communication style. Key Responsibilities: • Conduct telephone outreach to high-risk members and healthcare providers to close open preventive care gaps. • Support quality initiatives and seasonal call campaigns in coordination with regional market strategies. • Enter member health assessments, authorizations, and call documentation accurately into tracking databases. • Maintain the confidentiality and integrity of all Protected Health Information (PHI) in accordance with HIPAA standards. • Collaborate daily with remote care advocates and coordinators to meet health outreach goals. Qualifications & Skills: • High school diploma or GED equivalent required. • 2+ years of medical administration experience in a managed care, clinic, or medical office setting. • In-depth knowledge of HEDIS measures and quality care metrics. • Solid background in Medicare guidelines and healthcare claims processes. • Working knowledge of medical terminology and electronic health records. • Excellent verbal communication, active listening, and accurate data entry skills.
