This role focuses on home health physical therapy services, utilizing specialized skills to provide patient care based on physician orders. Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations with the appropriate tools and procedures
• Review patient records for diagnosis, history, precautions, and contraindications
• Establish goals and treatment plans for patient independence/function
• Re-assess treatment plan effectiveness and patient progress
• Communicate patient needs and progress to treatment team and family
• Consult with medical providers regarding patient evaluation and discharge
• Recommend and secure adaptive equipment for patients
• Maintain and submit required documentation following regulations
• Supervise Physical Therapist Assistants and Aides in patient care
• Attend and participate in staff development and educational programs

Requirements of the Physical Therapist

• Valid Physical Therapy license in the state of Arkansas
• Experience with HCHB and OASIS
• Home Health experience
