Physical Therapist | Home Visits - Grand Rapids | Full Time & Part Time

Schedule & Location:

• Now Offering: $5,000 Sign-on Bonus
• Days: Full & Part-time positions available
• Shift: Day shift (8 AM - 5 PM)
• Weekend Rotation: Every 7-8 weekends, no on-call
• Territory: Kent County

Atrio Home Care, a trusted partnership between Holland Home, Brio and Resthaven, is seeking a compassionate and motivated Physical Therapist to join our Grand Rapids team, serving clients within a 40 mile radius. This role offers the perfect blend of autonomy, flexibility, and purpose-ideal for someone who values personalized care and work-life balance. We are currently have full and part time opportunities available.

What You'll Do:

• Provide one-on-one in-home physical therapy services
• Conduct evaluations and develop individualized treatment plans
• Collaborate with physicians and interdisciplinary team members
• Help clients regain mobility, strength, and independence
• Document progress and support discharge planning
• Educate clients, caregivers and families, on therapy goals and techniques

What You Bring:

• Master's degree in Physical Therapy (required)
• Michigan PT license (required)
• 1+ year of PT experience (home health preferred)
• Geriatric experience highly valued
• Reliable transportation, valid driver's license, and auto insurance
