Hartford Healthcare HomeCare seeks a Weekend On-Call Visiting Nurse to deliver high-quality home health services across our Connecticut network. In this role, you provide after-hours and weekend nursing visits, conduct phone triage, and respond to urgent patient needs. Responsibilities include assessment, medication management, wound care, IV therapies, and coordination with physicians and the interdisciplinary team. You’ll document care in our EHR, educate patients and caregivers, and promote safe transitions.
