In this role, you’ll provide physical therapy services to the adult/geriatric population , working closely with patients to improve independence, mobility, safety, and overall quality of life . Responsibilities

• Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
• Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults
• Help patients restore functional mobility and safely perform daily activities with greater confidence and independence
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care
• Track patient progress, maintain accurate clinical documentation , and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications:

• Licensed Physical Therapist (PT) or license eligible in the state of practice
• Graduate of an accredited Physical Therapist Program
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults
