Registered Nurse (PRN)
Affinity Home Health Care Inc
Join as a Registered Nurse (RN) in a compassionate home health setting, providing vital case management, assessments, and coordination of patient care in clients' homes. Key Responsibilities Review client referrals and perform assessments to ensure appropriate care. Develop and implement individualized care plans. Direct, supervise, and respond to in-home care situations independently. Ensure compliance with agency policies, procedures, and professional standards. Perform additional duties as assigned by the Director of Clinical Services. Qualifications & Skills Graduate of an accredited nursing program; Baccalaureate preferred. Current RN license in Colorado. Minimum two years of nursing experience, including long-term, dementia, or acute care; home care experience preferred. CPR certification.
