Care Manager

We are seeking a Care Manager to serve clients and caregivers across St. Louis & St. Charles Counties, Missouri. The Care Manager is the clinical and relational anchor for a caseload of clients. You will conduct in-home assessments, build the care plans that guide every shift, coach the caregivers delivering that care, and stay close to clients and families so problems get solved before they become problems. What You'll Do Deliver Exceptional Client Care Conduct comprehensive in-home client assessments and reassessments. Develop and maintain individualized care plans. Ensure documentation, compliance, and safety standards are met. Conduct proactive client satisfaction check-ins. Address and resolve service concerns quickly and thoughtfully. You will set the standard for quality from day one. Support & Coach the Care Team Supervise and coach caregivers ("Weavers" aka our Home Care Aides). Introduce new Weavers to their clients, routines, and care plans. Provide field coaching, supervisory visits, and performance feedback. Reinforce Woven's high training and reliability standards. Partner with scheduling staff to ensure dependable coverage. Great care starts with well-supported caregivers. Keep Clients, Families, and Care Connected Serve as the trusted point of contact for clients and their families. Communicate changes in condition and adjust care plans as needs evolve. Coordinate with a client's broader care team — physicians, home health, and hospice — at transitions. Keep client records current and accurate in Woven's care platform. Families should always know their loved one is in good hands. Represent Woven in the Community Build referral relationships with hospitals, rehab centers, physicians, senior living communities, and elder care professionals. Conduct in-services and educational events. Attend community events and represent Woven publicly. Respond rapidly to referrals and convert assessments into long-term clients. Maintain disciplined CRM tracking and outreach cadence. You will be the face of Woven in the communities you serve. What Success Looks Like Within your first 90 days, you will: Know every client on your caseload — their care plan, their routine, and what a good day looks like for them. Complete assessments and care plan updates accurately and on time. Have caregivers who feel prepared, supported, and confident on every shift. Resolve service concerns before they become problems, with high client satisfaction. Keep documentation complete, compliant, and audit-ready. Sustain strong client retention and continuity of care. Who You Are Clinically competent and confident conducting in-home assessments. Warm and steady with clients and their families. A natural coach who lifts up the caregivers around you. Relationship-oriented and strong in community-facing environments. Highly organized and disciplined with systems and documentation. Calm under pressure and decisive in problem-solving. Comfortable spending your days in the field, visiting clients across St. Louis County. This role is ideal for someone who wants to know their clients by name, not by case number. Qualifications Bachelor's degree preferred. RN, LPN, PT, or MSW license preferred. 1+ years of care management, case management, or supervisory experience in healthcare or service-based environments. Experience working with older adults. Strong communication, coaching, and documentation skills. Comfortable with technology — care plans, schedules, and notes live in our platform. Reliable transportation and valid driver's license. Preferred: Sales or marketing experience. Experience in home care, home health, or senior services strongly preferred.
