Case Manager
We are seeking a dedicated and experienced Case Manager to join our team. This position requires a strong background in acute care and will involve working with patients to ensure they receive comprehensive care throughout their treatment journey.
Job Responsibilities
- Manage patient cases from admission to discharge.
- Coordinate with healthcare providers to develop and implement care plans.
- Conduct assessments and evaluate patients' needs.
- Work collaboratively with multidisciplinary teams to improve patient outcomes.
- Maintain accurate and timely documentation in Epic.
Working Conditions
- Day shift: 5x8 hours (08:00 - 16:30).
- Required to work every other weekend.
- This role may involve floating between similar units based on patient care needs.
- Local candidates are encouraged to apply.
Qualifications
- Minimum of 2 years of acute care experience required.
- First-time travelers accepted provided they meet screening approval.
- Excellent communication and organizational skills.
