SMH LPN - Long Term Care

Shift: D/E/N Rotating shifts, including weekends, holidays, on-call, and callback as needed

Job Description:

Opportunity to provide care for older adults in a long term care environment. Duties include direct patient care including medication administration and oversight of CNA colleagues. Must have good critical thinking skills and ability to work independently while providing care with dignity and respect. At least 2 years experience in long term care setting, NYS LPN license and BLS from AHA required.

Responsibilities:

• Direct patient care for 8-10 residents per LPN
• Medication administration
• Oversight of CNA colleagues
• Provide care with dignity and respect

Requirements:

• NY LPN license
• BLS certification (American Heart Association only)
• Minimum of 2 years' experience in a long term care setting
