Licensed Practical Nurse- Day Shift

Pay Range $29.00–$33.67/hour (based on experience)

Sign-On Bonus: Up to $3,000

Rate : Full-Time, Days

Join the compassionate and skilled team at Taconic Rehabilitation and Healthcare Facility at Beacon , where we provide both long-term care and short-term rehab in a supportive, resident-centered environment.

• Deliver nursing care under the supervision of an RN in a skilled nursing and rehab setting

• Administer medications and treatments per physician orders and care plans

• Monitor vital signs, symptoms, and changes in condition

• Maintain accurate clinical documentation using electronic medical records (EMR)

• Support post-acute care and recovery goals for short-term rehab residents

• Participate in care plan meetings and collaborate with interdisciplinary teams

• Uphold infection control and resident safety protocols

• Valid LPN

• New graduates encouraged to apply – training available

• Prior experience in long-term care , rehabilitation , or skilled nursing preferred

• Strong clinical, documentation, and communication skills
