Pharmacy Care Coordinator

Supporting clinical outreach and medication management, the full-time Pharmacy Care Coordinator will manage an assigned caseload, engage with members and providers, and facilitate care coordination remotely. Key responsibilities Conduct outbound calls and utilize portal chat to schedule appointments for medication consultations Coordinate follow-ups with providers, pharmacies, and members to address medication recommendations and concerns Assist members in navigating their health benefits and participate in team activities and special projects Required qualifications At least 3 years of work experience in health care or retail pharmacy Active National PTCB Certification Experience making a high volume of outbound calls to members, providers, and pharmacies Ability to manage daily responsibilities and tasks independently Flexibility to shift between tasks, projects, and priorities frequently
