MDS Coordinator

Ciena Healthcare is seeking an MDS Coordinator RN to lead resident assessment and care planning in our skilled nursing and rehab center. This role oversees accurate and timely MDS 3.0 completion, coordinates interdisciplinary care plans, and ensures compliance with Medicare/Medicaid and state regulations. You’ll partner with nursing and therapy teams to support optimal clinical outcomes, reimbursement accuracy, and quality measures.
