At Hospice of Orange & Sullivan Counties Inc, our clinical team serves the community we live in, our neighbors. OPEN CALL FOR REGISTERED NURGESWhat are we looking for? The RN Case Manager is essential to the Hospice organization. From admission to the end of our patients' journeys, our RN Case Managers provide professional nursing care by utilizing all elements of the nursing process. We are looking for RN Case Managers to join our field team as leaders in our local community. Our RN Case Managers lead a designated team that includes an LPN and Hospice Aide Specialist who are assigned to their own personal territory located throughout Orange and Sullivan Counties. In addition to providing hands on medical care, the RN Case Managers collaborate with the Social Work and Spiritual Care teams to develop a comprehensive plan of care.
