Nurse Practitioner

Nurse Practitioner Opportunity in Kissimmee, FL

Pride Health is hiring a Nurse Practitioner in near Kissimmee, FL! Working between two LTC facilities (Kissimmee and St Cloud). Schedule: Monday-Friday (8am-5pm). Contract: 4-6 month contract. Pay Rate: $65-70/hour.

Specific Duties:

• Provide specialized care to high-risk health plan members
• Work as part of an interdisciplinary team
• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate documentation, including ICD-10 coding
• Communicate and collaborate with the care team
• Facilitate advanced illness and care planning discussions
• Offer counseling and education to patients and caregivers

Must have qualifications/experience:

• Nurse Practitioner or Physician Assistant
• NP Certified in one of the following (FNP, ANP, GNP)
• Access to transportation
• 1+ year of experience (LTC, SNF, Geriatric preferred)
