\n ?? Position Overview

\n We are seeking an experienced Physical Therapist (PT) to provide compassionate, patient-centered rehabilitation services in a Skilled Nursing Facility setting. The ideal candidate will help patients improve mobility, strength, functional independence, and overall quality of life through individualized treatment plans.

\n ? Requirements

\n ?? Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
\n ?? Active California PT License required
\n ?? Minimum 3 years of Physical Therapy experience preferred
\n ?? Skilled Nursing Facility or similar rehabilitation experience preferred
\n ?? Strong knowledge of therapeutic exercises, modalities, and adaptive equipment
\n ?? Excellent communication and interdisciplinary collaboration skills
\n ?? Compassionate, patient-focused approach to care

\n ?? Responsibilities

\n ?? Perform comprehensive physical therapy evaluations
\n ?? Develop and implement individualized treatment plans
\n ?? Provide therapeutic interventions to improve strength, mobility, and function
\n ?? Monitor patient progress and modify treatment plans as needed
\n ?? Educate patients and families on exercises, safe mobility, and home modifications
\n ?? Collaborate with nurses, physicians, occupational therapists, and other members of the care team
\n ?? Maintain accurate and timely clinical documentation
\n ?? Ensure treatment plans comply with facility policies and regulatory standards
