Position: Licensed Practical Nurse - LPN - Home Health Location: Massillon, OH Job Id: 12864 # of Openings: 1 Overview Absolute Home Health is currently seeking a compassionate Skilled Licensed Practical Nurse (LPN) to assist clients one-on-one in their homes. Our goal is to help patients transition from hospital to home and provide continuous care to enable them to remain with their loved ones. Absolute Home Health offers skilled nursing, therapy services, social services, and home health aides in the home under the direction of the patient’s primary care physician. Our LPNs are an essential part of the team, providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with team members to help achieve positive patient outcomes. Responsibilities Work well in a team environment and independently Demonstrates flexible and efficient time management and ability to prioritize workload Computerized medical records Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care Adherence to policies and procedures Helps achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy OASIS knowledge and competency 2 years of skilled home care experience, preferred Excellent written and oral communication skills Qualifications 2 years of skilled home care experience, preferred.
