Compensation:

$123,000 to $150,000 annually, plus performance-based incentives

Bookmark Medical is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our established primary care teams in the East Valley of AZ, floating between our clinics in Mesa, Gilbert, and Superstition Springs.

What You ll Do
Provide comprehensive primary care services, including preventive care, acute visits, and chronic disease management.
Perform patient evaluations, physical examinations, and appropriate diagnostic assessments.
Develop and implement individualized treatment plans based on clinical findings and patient needs.
Order, review, and interpret diagnostic tests and laboratory results.
Educate patients on preventive health, disease management, and treatment options.
Collaborate with physicians, specialists, and other members of the care team to coordinate patient care.
Maintain timely, accurate, and complete documentation in the electronic health record.
Follow established clinical guidelines and evidence-based practices to support quality patient outcomes.

Qualifications
Successful completion of the educational requirements for a Nurse Practitioner or Physician Assistant through an accredited program.
Current, unrestricted NP or PA license in the state of practice, or eligibility to obtain licensure prior to employment.
Minimum of one year of clinical experience as an NP or PA in primary care.
Strong clinical judgment, communication skills, and commitment to patient-centered care.
Ability to work collaboratively with providers and clinical staff in a team-based primary care setting.
Experience using electronic health record systems.
