Physical Therapist (PT) – Full-Time Civita Care Center at Salmon Brook | Glastonbury, CT $45–$50/hour | Full-Time | Benefits Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center at Salmon Brook. This full-time opportunity is ideal for a clinician who enjoys helping residents restore mobility, build strength, and achieve greater functional independence in a skilled nursing and post-acute care setting. What You'll Do Perform physical therapy evaluations and develop individualized plans of care Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility Establish resident-centered goals and monitor progress throughout the rehabilitation process Modify treatment approaches based on clinical findings and resident response Educate residents and caregivers on exercises, mobility techniques, and safety strategies Complete accurate and timely clinical documentation Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team What We Offer $45–$50/hour Full-time position Medical, dental, and vision insurance 401(k) Paid time off and holiday pay Continuing education support Supportive and collaborative rehabilitation team Short-term rehabilitation and long-term care caseload Opportunities for professional growth Qualifications Active Connecticut Physical Therapist license required Graduate of an accredited Physical Therapy program Strong clinical judgment, communication, and teamwork skills Skilled nursing or rehabilitation experience preferred, but not required New graduates are welcome to apply #J-18808-Ljbffr Post Rehabilitation Services
