Regional MDS - Skilled Nursing (Maryland & Virginia Region)

The Regional MDS role is a cornerstone of our operational model, responsible for overseeing MDS operations across five skilled nursing facilities, driving clinical and financial outcomes through hands-on leadership, mentorship, and strategic oversight. This role requires a leader who is comfortable being present in the buildings, actively engaging with MDS Coordinators, Directors of Nursing, Administrators, and clinical staff to ensure accurate, timely, and compliant assessments. Four of the facilities are in the Baltimore suburbs, with one additional site in Virginia Beach that requires occasional travel, making this position ideal for a professional with regional experience and the ability to balance multiple priorities across diverse locations. The ideal candidate will not only ensure regulatory compliance and optimize reimbursement performance but also foster consistency, strengthen staff capabilities, and standardize best practices across the portfolio. This is a role for someone who thrives on impact, wants to be in the field rather than behind a desk, and understands the critical link between high-quality documentation, accurate MDS reporting, and the financial and operational health of skilled nursing facilities.
Responsibilities
  • You will oversee MDS operations across five skilled nursing facilities, ensuring that all assessments are accurate, timely, and compliant with OBRA and PDPM requirements.
  • You will conduct regular onsite audits of each facility's MDS processes and implement corrective action plans when deficiencies are identified.
  • You will monitor Case Mix Index (CMI) trends and implement strategies to improve reimbursement outcomes across all facilities.
  • You will partner closely with facility leadership, including MDS Coordinators, Directors of Nursing, and Administrators, to strengthen clinical documentation and overall staff performance.
  • You will provide hands-on mentorship and ongoing training to facility-level MDS staff to ensure consistent understanding and execution of best practices.
  • You will ensure that each facility is prepared for surveys and audits, providing support and guidance as needed.
  • You will standardize processes, tools, and workflows across all facilities to create consistency and efficiency in MDS operations.
  • You will track performance metrics, analyze trends, and recommend actionable improvements to optimize both clinical and financial outcomes.
Qualifications
  • You have 3-5+ years of regional MDS experience in skilled nursing.
  • You possess a strong knowledge of PDPM, OBRA regulations, and reimbursement practices.• You have a proven track record of improving CMI and maintaining compliance across multiple facilities.
  • You are comfortable working onsite and building strong, collaborative relationships with clinical and administrative teams.
  • You are willing to travel regularly in the Baltimore area and occasionally to Virginia Beach.
  • An RN license is required.
Compensation
  • You will receive a base salary of $125,000 - $160,000, commensurate with experience.
