Founding NP for Telehealth & In-Home Primary Care

Together Health

Together Health in Little Rock, AR is seeking a founding Advanced Practice Provider (NP/PA) for in‑home and telehealth primary care. The contractor/PRN role focuses on delivering holistic care to adults across telehealth, in-office, and home visits in and around Little Rock.

You will collaborate with the clinical team to develop individualized care plans, coordinate care, interpret diagnostics, and help shape protocols as we expand regionally.
