The Wound Care Nurse (RN) oversees the facility infection control and wound care programs. Conducts wound care rounds with the facility wound care provider, collaborates with the interdisciplinary team and clinical staff to establish and maintain treatment programs. Gathers and maintains the facility infection control program.Key ContributionsConsults and coordinates with the interdisciplinary team (IDT) and healthcare professionals to plan, implement and evaluate residents wound care treatments and monitor the residents progress and clinical needs.Participates in quality committees including quality assurance and performance improvement (QAPI), infection control, safety, risk management, etc.Leads the identification and mitigation for resident risk factors for pressure ulcers/injuries.Assists the interdisciplinary team in identifying and managing resident pain related to wounds.Provides skin and wound care and treatments, as ordered.Rounds weekly or more frequently as needed with the facility wound care provider or facility leadership.Provides information and care instructions to residents and families related to their specific condition(s) and treatments(s) in a language and format that can be understood by the resident/family.Provides guidance, education, and oversight to the facility staff regarding wound care practices and treatments.Evaluates pressure-relieving devices, therapies, dressings and wound care products and make recommendations to the interdisciplinary team regarding their efficacy and use.Coordinates, implements, monitors, and updates the infection prevention and control program in accordance with current regulations and guidelines governing skilled nursing facilities.Monitors and reports compliance with the IPC program to the administrator and QAPI committee.Ensures the facility is compliant with current Centers for Disease Control and Prevention (CDC), Centers for Medicare and Medicaid Services (CMS), Occupational Safety and Health Administration (OSHA), and local regulations concerning infection prevention and control.Monitors and ensures all personnel follow established infection prevention and control practices and techniques when caring for residents.Establishes and maintains an infection surveillance program based on standardized definitions of infections.Reviews, summarizes and reports data related to key infection prevention and control initiatives including antibiotic stewardship, healthcare acquired infections, immunization programs, and outbreaks.Reports all reportable diseases to the county and state health departments.Completes any reportable information to state or federal government organizations related to resident vaccination, infections, etc.Assists with the coordination of residents' flu shots.Qualifications & RequirementsExcellent oral and written communication skills in the English language required.During emergencies or natural disasters, may be required to remain on duty for the duration of the event.Associate degree or bachelor's degree in nursing from an accredited program requiredActive Florida Registered Nurse licensure requiredActive Florida CPR Certification is requiredMinimum of 2 years' experience in a long-term care setting. Requires flexible schedule.Must be able to lift up to 40 pounds alone & assist in moving a resident.Infection Control certification requiredWound care certified or willing to become certified within 90 days
