Mobile Crisis Clinician - Emergency Services

Tasks/Role:

• To provide individuals/families in need with 24-hour crisis intervention and stabilization, to prevent or reduce potential suicide, serious debilitation, violent behavior and decrease the need for long-term hospitalization.

• Work in close collaboration and cooperation with other Emergency Service members and other multidisciplinary agency staff, in carrying out team and agency policies/procedures. For mobile clinicians, this includes effectively working with ES clinicians embedded at the Exeter Hospital to create a seamless transition for the patient when that level of care is needed.

• Provide phone, in-person or telehealth crisis intervention, assessment and stabilization services in a patient’s home, community setting or other appropriate setting during agency open hours and/or after-hours. At times, this will include short term crisis counseling and case management.

• Participate in 24-hour (after hours) phone and face-to-face crisis services at The Exeter Hospital.

• Assess client crisis state/immediate needs; level of urgency and selecting mode of intervention in relation to suicide risk, violence potential, current level of coping, mental status, support system, need for medication, crisis housing, brief hospitalization, IEA or referral for outpatient treatment. In this way, the ES clinician manages the acute phase of clients presenting in behavioral crisis. The scope of practice with include working with youth, adult and geriatric populations.

• Provide consultation to staff at the local homeless shelters.

• Formulate and initiate plan of action independently and/or in consultation with the client, psychiatrist, case manager, various other staff as appropriate to situation.

• Utilize supervision effectively by sharing pertinent information and accepting feedback with a goal for continued clinical improvement through employment with the agency.

• Provide assistance to colleagues in order to optimize the delivery of services to the patient population. Always maintains professional demeanor.

• Communicate information to appropriate staff, regarding client crisis status and interventions, necessity for follow-up and treatment planning, including development and use of Emergency Services treatment plans and approach for specific clients.

• Assume case management and psychotherapy functions when performing crisis stabilization with emphasis on returning patients to their previous baseline by facilitating connections to services within agency or community.

• Consultation and/or evaluation with Exeter Hospital Emergency Room physicians and staff, for persons presenting in the ER with psychiatric problems. When requested, telephone consultations with Portsmouth Regional Hospital Emergency Room for Seacoast clients who present there in crisis and/or who require a Conditional Discharge Revocation. At times, consulting and evaluating those who present in other parts of the Exeter Hospital other than the emergency department and medical floors may occur.

• When ordered by agency psychiatrist or APRN, completion of Conditional Discharge Revocation at either Exeter Hospital or Portsmouth Regional Hospital.

• Provide emergency consultations/information and evaluation to various other community agencies and organizations, such as police, jail, foster home, etc., and homeless shelters in the area when appropriate. This may include out of catchment interactions as determined by the crisis call center in the state.

• Consultation with Seacoast Lodge Staff regarding residents in crisis, offering support to lodge staff, making crisis evaluation of residents when necessary (after hours & when Center is closed).
  Willingness to transport clients to an emergency department, crisis center or peer support when deemed clinically appropriate.

• Willingness to work non-traditional shifts in order to staff 24/7 program requirements.

• Keep appropriate records and documentation as outlined in ES and agency policies and procedures. Documentation should always be timely and accurate within the standards set forth by the agency.

Specific Education/Experience Required:

Master's Degree in clinically related mental health discipline with significant experience in clinical diagnosis and therapeutic intervention. Ability to form rapid rapport and interact with broad range of clients, including severe and chronic populations. Ability to interface with various community agencies/organizations and multidisciplinary staff. CPR and First Aid Training are recommended. 40-hour crisis response training that includes concepts and topics in accordance with the National Guidelines for Crisis Care Best Practice Toolkit. Columbia Suicide Severity Rating Scale.
