ACT Mental Health Clinician - Case Management & Therapy

Horizon House, Inc.

Horizon House, Inc. seeks a dedicated ACT team member in Delaware County to provide service coordination for a group of clients, coordinating with the treatment team and implementing individual treatment plans. The role includes on-call crisis intervention, client advocacy, and family education to support rights and preferences. The position requires a Master’s Degree in a related field with 1–2 years of direct MH experience; CAC certification is preferred, and valid driver’s license is expected. #J-18808-Ljbffr Horizon House, Inc.
